Building forms
Design and evaluate public forms for data capture
With the form builder you create your own forms – for contact enquiries, applications, surveys or lead generation.
Create a form
Open forms
Navigate to Automation → Forms and click New form.
Add fields
Via drag & drop you add fields: text, email, select, multiple-choice, date, file upload, consent checkboxes – and an appointment field with which customers can book a free slot right in the form (→ Calendar & appointments).
Define logic
With conditional fields you show fields only depending on previous answers.
Publish
Publish the form and get a public URL, a QR code and embed codes (see Embed a form).
Embed a form
A published form can be built directly into any website. In the form's action menu, open Embed and copy one of the two codes:
Script widget (recommended) – adjusts the height automatically to the content, no scrollbar in the frame:
<div data-simpliserv-form="TOKEN" data-simpliserv-slug="SLUG"></div>
<script src="https://simpliserv.app/embed.js" async></script>Simple iframe – without a script, with a fixed height (adjust min-height if needed):
<iframe src="https://simpliserv.app/f/SLUG/TOKEN?embed=1"
style="width:100%;border:0;min-height:800px"></iframe>Changes to the form appear in the embedded form after publishing, on the next load. File uploads as well as appointment and signature fields work in the embedded form just like on the public page.
You can measure successful submissions in the embedded form with Google Tag Manager or similar – see Track form conversions (GTM).
Restrict embedding to your own domains
By default the form can be embedded on any website. Under Settings → Security → Restrict embedding you enable the restriction and enter under Allowed domains the domains on which the form may appear – one per entry, e.g. example.com. Subdomain wildcards like *.example.com are supported. On all other domains the browser blocks the embedding. The restriction takes effect after the next publish.
Evaluate responses
In the Submissions tab you see every response – with filter, search and export functions. Engagement sessions show how far users filled in the form.
Link workflows
Incoming responses can automatically:
- create a contact
- create a task
- send a confirmation email
- start a workflow
Analytics
Statistics like conversion rate, average processing time and drop-off points support optimization.
