Customize the service registration form
Configure the fields, copy and logic of the public service registration form
→ Open the service registration module
You can adapt the public service registration form to your business model – for repair intake, maintenance requests or device reports.
How to embed or link the form is explained in the guide Service registration.
Where are changes made?
You'll find all settings in the admin area under Administration → Modules → Service Registrations.
Configure fields
Show or hide standard fields
In the module settings you enable or disable standard fields such as salutation, address, device type, serial number or issue description.
Add custom fields
Via custom fields you define additional fields (text, select, date, file upload). They appear automatically in the public form.
Set required fields
Per field you can set whether it must be filled in. That ensures all relevant information comes through.
Adjust the order
Fields can be reordered via drag & drop to shape the sign-up process logically.
Copy and branding
- Customize the welcome and closing text to greet users.
- Logo and colours come from the organisation settings and are applied automatically.
- Privacy notices and a terms checkbox can be shown as mandatory.
Automation
Incoming service registrations can trigger workflows, e.g.:
- Send a confirmation email to the customer
- Create a task for the workshop team
- Assign the entry to an employee automatically
For this, use the "New service registration" trigger in the workflow builder.
Email template
The confirmation and status emails are maintained in the email templates under the "Service Registration" area. Variables like {{customer_name}} or {{ticket_number}} are available.
Public status page
Every registration automatically gets a tracking URL. Customers see the current status without logging in. You can name the displayed status stages individually in the module.

How is this guide?
Handle service requests
Review incoming service requests, accept or reject them, convert them into an order or estimate, schedule appointments and capture them manually
Public order registration
Customers create an order directly without logging in: contact details, product and the requested work. SimpliServ immediately creates an order, assigns the customer and starts the customer flow.
